Medarot R is the first Medarot game to employ 3D graphics, specifically for its battle sequences. It uses the character designs and feature some characters and Medarots introduced in the [[Medarotter Rintarou!]] manga although its storyline does not closely match that of the manga.

The game presents an alternate retelling of Ikki's story, placing it in a different continuity from Medarot 2, 3, and 4. However, it still serves as a sequel to the [[Medarot 1|original game]] and vaguely to the [[Medarotter Rintarou!]] manga as well.

* The story is still about Ikki facing RoboRobo Gang, but the events of the chapters are completely different from the events of Medarot 2.
 
* The characters have undergone a redesign, some just have a different color scheme but others are practically unrecognizable.
 
* Ikki necessarily starts with Metabee, instead of the player having the choice between him and Rokusho.
** However, Kuwagata Medal is given by Hikaru at the end of the third chapter and at the beginning of the next chapter, Rokusho's parts are given to Ikki by Katekin Ten, making Ikki have both medarots in a single game.
 
* The game doesn't have side-quests other than looking for hidden medals, so there's no choice between Arika and Karin with the entire script being very linear.
 
* All medals can be obtained in a single playthrough as there aren't two versions.
 
* All leaders of the RoboRobo Gang from Medarot 2 have been replaced by new characters although they have some equivalence.
** Shiokara was replaced by Red Makko.
** Sarami was replaced by Green Pork.
