There were 102 new Medarots created for Medarot R. The game also contains the 60 Medarots from Medarot 1 and 91 from Medarot 2, bringing the grand total up to 253.

Continuity and Placement[edit]

The game presents an alternate retelling of Ikki's story, placing it in a different continuity from Medarot 2, 3, and 4. However, it still serves as a sequel to the original game and vaguely to the Medarotter Rintarou! manga as well.

  • Character Redesign: Characters have been redesigned, with some receiving only a new color scheme.
  • Starter Medarot: Ikki can only starts with Metabee, removing the player's choice between him and Rokusho.
    • However, the Kuwagata Medal is given by Hikaru at the end of the third chapter, and at the beginning of the next chapter, Rokusho's parts are provided to Ikki by Katekin Ten, allowing Ikki to have both Medarots in a single game.
  • Side-Quest Changes: The game lacks side-quests aside from searching for hidden medals, making the script very linear with no choice between Arika and Karin.
  • Medal Collection: All medals can be obtained in a single playthrough as there are no separate versions.
  • RoboRobo Gang Leaders: All leaders from Medarot 2 have been replaced by new characters with some equivalence:
    • Shiokara was replaced by Red Makko.
    • Salami was replaced by Green Pork.
    • Surume was replaced by Pink Duck.
    • Sakekaasu was replaced by Silver Bat.
    • Dr. Hebereke was replaced by Medal God.
    • Katekin Ten has a role similar to the scientist at the end of Medarot 2/Core who releases Primity Baby.
  • Tournament: The Island tournament from Medarot 2 was replaced by the Medarot Tournament and Hyper Tournament.
  • Battle System: The game does not use the traditional "Relay Race" format seen in most Medarot RPGs. Instead, a flashing icon indicates how close a Medarot on either side is to performing an action.
  • New Characters: This is the first game to introduce characters from the Medarotter Rintarou! manga. However, in the main continuity, this introduction occurs in Medarot 3.
    • Medarot 2 Core includes Rintarou, making it the second game after Medarot R to feature the character in a retelling of Medarot 2 events.
